Pros: Very comfortable - stretches your back out. Can it cause a bit of "pain?"
Cons: Expensive, Gets "pilly." Box spring/foundation failing after 3 years.
The Bottom Line: Really comfortable mattress - though gets "pilly" with use - is the cover wearing out? Competition offers less expensive options - so shop around! (Mar 2004)

We purchased a tempur-pedic mattress about 6 years ago and have been very happy with the purchase. Although expensive, it has seemed to be worth the money (not a bargain, but a fair price).
We had a Queen sized bed, and didn't like how when one of us moved, or shifted around, the other was disrupted or woken up. Given the small size of our house and bedrooms, a Queen sized bed was the largest we could go since we have found when traveling, a King sized bed offers tremendous isolation.
We were impressed at the variety of mattress systems on the market, many of which offered pretty good isolation even when Queen sized, but a lot didn't seem terribly comfortable. We were about to purchase one anyway, and found out about the tempurpedic mattress when at a "Relax the Back" store. We were impressed with the mattress, less the store (we didn't buy it from them). After digging into the technology, the generous guarantees, we took the plunge and bought one over the phone (I find the ads kind of dorky, and the helpful staff like to pretend to be Swedish which I found bit silly and not required).
First impressions
The mattress arrived and we got it carted into the house. Setup was tough - the mattress is difficult to maneuver, and took the both of us. We got the box springs and mattress set up and had the first night. Wow, the mattress was firm and soft at the same time (as advertised), and we woke up with our backs feeling all springy and stretched out. We did notice that the temperature of the bed was warmer than with our old mattress, but taking off 1 layer of blanket solved that problem. We also could let the house get colder in the winter nights which might save energy, though to be honest, we didn't track it.
6 months later
After about 6 months, we got very used to it. We were questioning if it still worked. We did the hand test, and it seems to have kept its molding ability. And a business trip convinced me that I had just got used to the comfort - as the great bed at the hotel I stayed in, just didn't feel that good, though it was the same brand I had owned previously! When I got back home - the STRETCH sensation was there again on the first night.
The mattress seems to have held up for the last 4 years. I have no complaints, and would probably buy it again if I needed to. The springiness isn't quite as good as it was when I first got it, but completely acceptable (still does the hand test). In winter, when it is colder, the mattress is harder, and in summer, it is softer, but overall, it is very comfortable.
I forgot to mention one other thing - I don't have to go to the chiropractor nearly as much as I did before. It seems like most back problems tend to right themselves after a night or two on the bed, rather than get worse as it did before. I don't know if this will solve all back problems, but for mine, it seems to help.
CONCLUSION
What else? I think this is a great mattress, I doubt I will choose to sleep on anything else at home. It is expensive as far as mattresses and box springs go, but I think it is at a fair price for what you get (SEE UPDATE).

Durability: 3 (Older models hold up great, new models seem to be doing well, but gets "Pilly" between the cover and foam rubber - is the cover prematurely wearing out?? Box spring has developed squeak/rubbing sound - could it be failing? **Reduced from 3 to 2**)
Ease of Use: 4 (Setup - make sure to have a hubby or friend to help you!)
Style: 4 (As stylish as a MATTRESS gets!)
UPDATE (10/12/2003)
We moved and got a larger bedroom that allowed us to get a king sized bed (!). We looked at mattresses again - and saw that both conventional mattresses as well as tempur style mattresses have increased in both quantity and quality since our last purchase. It appears that both cheaper and quality competition has entered the fray - which is a boon to those shopping more recently! The person we bought from gave us a discount (though we were informed later that they weren't supposed to discount - though they were willing to honor the discount they had quoted us) so it made it really easy to stick with this brand.
Without that discount we got - Tempurpedic seemed to be ~10% more expensive than the competition - I am not sure we would have selected them to buy given how good alternatives seemed. My advice would be that if you like the style and feel of the mattress, shop around. Tempurpedic has a "patent" on their mattress - but that seems to be their having a hard piece of foam on top of the tempur material - which didn't seem to affect overall comfort from what I can tell. Like I said, before paying the premium, make sure you think it is money well spent and shop around.
We are still very happy with Tempurpedic, but the number and quality of the competition makes the choices much more difficult!
UPDATE (March 2004)
We have begun our Spring cleaning - and noticed that the new mattress has generated a number of "pills" between the removable top cover and the cloth mattress cover. At first we were alarmed and thought the mattress was possibly disintegrating - but upon closer investigation the little lumps were large pills that had formed from the cloth over the last 6 months of ownership. I think the foam rubber rubbing against the cloth is probably causing this phenomenon? Looks like we will have to clean them out at some point, though they do not seem to reduce comfort.
The major innovation of having a removable, washable cover seems to have a side effect that I hope Tempurpedic will fix, because while it doesn't seem to reduce comfort, it is rather messy and might mean the cloth mattress cover is going to prematurely wear out! I reduced my durability rating by 1 bar for this.
UPDATED (September 2005)
We cleaned out the pills in in the mattress - it appears to be solely due to the cover rubbing against the foam top of the mattress. After using a crevice tool on a vacuum to remove the pills, they haven't come back. Must be a break-in thing - they weren't the foam, but the cover, which is a relief!
The mattress is holding up well, and is still one of the most comfortable mattresses we own. However - it appears that the older Queen sized mattress (now in a guest room) is a bit firmer than the King sized, and newer one we purchased.
Since the original review and a couple of trips to our house in Western NY - my parents are currently trying out a Tempurpedic mattress in their house. They said they woke up with sore backs. Whether this a variation of the "springiness" my wife and I experienced, or something else remains to be seen. They are currently in the rather trial period with the mattress - and have rejected a popular adjustable air mattress brand in favor of trying this. I will drop 1 star on the comfort due to this experience of my parents.
UPDATED (September 2006)
The mattress has been doing very well, but over the last 3-4 months we have noticed that the box spring has developed a squeak/rubbing sound when weight is put on it - definitely coming from the box spring. The mattress itself is great, but the box spring apparently isn't built to the same quality as the mattress (it came as a set - king sized mattress and 2 box springs as the foundation). So, you should be wary of purchase, and we will downgrade durability by 1 point.
